Mindfulness meditation is one of the techniques where your mind is not only fully aware of the stillness but also keeps observing when your attention starts to wander from the object of the meditation (in most cases the object of meditation is the repetitious breath, flow of thoughts, emotions or feelings). And as you observe these objects with a sense of detachment, you will notice familiar patterns that may seem irrelevant to harbor and you may want to let go these patterns. Detachment or letting go of these patterns helps in cultivating equanimity in your daily life (Meditation and Equanimity). Thus, mindfulness meditation allows you to observe your habitual thought patterns and emotional responses and provides the opportunity to change them to more positive ones. Mindfulness meditation helps us become aware of what is already true at every moment, providing us with an opportunity to become more present with ourselves just as we are. This, in turn, helps us to overcome the unnecessary suffering that results from trying to escape the discomfort and pain that we experience in our daily lives.

Today’s discussion is about mindfulness meditation and its role in alleviating the acute pain response. Pain is an unpleasant sensory and emotional experience associated with actual or potential tissue damage and is part of the body’s inherent protective mechanism to motivate the individual to pull out from a damaging situation and prevent further damage to the tissue, allow the damaged tissue to rest and heal, and avoid similar encounters. Thus, the phenomenon of pain is beneficial in a true sense when, for example, your back sustains a blow and the injured tissues need care and protection (Understanding Pain). 

Just like inflammation, the phenomenon of pain is generally transient, and recedes when the stimulus is removed or the damaged tissue has healed (How and Why Yoga Helps with Pain). When your body encounters an acute pain stimulus, your brain releases a large number of pain reducing chemicals called opioids. Opioids act by attaching to specific proteins called opioid receptors, which are found in the brain, spinal cord, gastrointestinal tract, and other organs in the body. The binding of the endogenous opioids to their specific receptors results in reduction of the perception of pain. However, there are certain abnormal pathological conditions where pain is a constant feature and persists for a long time (chronic pain). While in some chronic cases the levels of the endogenous opioids are not sufficient to alleviate the pain response, in most cases the body becomes insensitive to the body’s levels of opioids. Chronic pain that lasts beyond the expected period of healing can interfere with a person's quality of life and general functioning. Unfortunately, chronic pain brings with it other health issues including constant fear, worry, anxiety, depression, and memory deficits and turns out to be the most common reason for individuals to visit their physician (Techniques for Managing Pain with Yoga). 

One of the methods of pain management is the external administration of opioids like vicodin, morphine, codeine, or oxycontin. These external opioids not only blunt the pain response, they also produce a euphoric response since they also affect the brain regions involved in reward. Regular or long-term use or abuse of opioids can lead to physical dependence and, in most cases, addiction. Naloxone is a drug that works as an antidote to the opioids and is mainly used to reverse an opioid overdose. Naloxone works by blocking the analgesic (anti-pain) and the euphoric response of the opioids in the system. Naloxone only works if a person has opioids in their system; the medication doesn't work on other drugs. 

Now what does all this have to do with mindfulness meditation? Well, we know that meditation produces relaxation and the lovely blissful feelings that result from release of tension. The question is: does meditation trigger the release of endogenous opioids which in turn produces the pain relief and the pleasant effects thereafter? This was the premise of a new study Mindfulness-Meditation-Based Pain Relief Is Not Mediated by Endogenous Opioids in which the researchers set out to ask a simple question, “does mindfulness meditation (MM) produce pain relief and if yes, then does it do so through the release of endogenous opioids”? 

The double blind randomized study involved 78 healthy, pain-free people who were randomly assigned to receive different interventions. The first group was trained in mindfulness meditation for 20 minutes a day for four days. And the second group, which served as a control group, spent the same amount of time listening to a book read aloud. The scientists then exposed all the participants to a pain stimulus by placing a hot probe on the back of their legs. The group that practiced mindfulness meditation experienced significantly reduced pain intensity and pain unpleasantness than the control group. 

The next thing to determine was if the pain reduction in the mindfulness meditation group was due to release of endogenous opioids. One way to test this was to block the release of endogenous opioids (if any) by administering the drug Naloxone, which would then reverse the beneficial analgesic effects of mindfulness meditation. (A control group was given simple saline as a placebo.) To the surprise of the researchers, the meditators who received the drug Naloxone before being subjected to the hot probe continued to experience significantly reduced pain intensity and pain unpleasantness than the control group, suggesting that Naloxone administration failed to reverse the meditation-induced pain relief. There were also no significant differences in pain intensity or pain unpleasantness reductions between the meditation + naloxone and the meditation + saline groups.

The findings clearly demonstrate that mindfulness meditation eases pain through some distinct pathways that are independent of the endogenous opioid mechanisms, suggesting that meditation-based pain relief does not require the role of endogenous opioids. The researchers do not have a clear explanation of the effect of mindfulness meditation on the brain that results in the pain relief. But based on their past results, they believe that meditation activates several regions of the brain that are associated with emotion regulation, focus, and cognitive control. At the same time, meditation also down-regulates the thalamic activity that transmits painful information from and to the body. Thus, the global effect of mindfulness meditation helps to prevent pain and painful information from spreading throughout the brain and body. 

It remains to be determined whether mindfulness meditation would be helpful in the treatment of chronic pain and also be effective in overcoming opiate drug abuse. For now though, mindfulness meditation seems to be a very attractive technique for acute pain relief and for those seeking a non-opiate pain therapy. Well, another group of researchers at  the same Institute of Optics at the University of Rochester, along with colleagues from several other research institutions have done it again. This time using their knowledge of applying adaptive optics to visualizing the much smaller photoreceptor rod cells present in the living eye.

This innovation, described in two papers in the Optical Society's (OSA) open access journal Biomedical Optics Express, will help doctors diagnose degenerative retinal eye disorders sooner, leading to quicker intervention and more effective treatments.

Adaptive optics technology likely to spur sight-saving interventions, usher in new era of eye disease research, diagnosis and treatment

Scientists today reported that the tiny light-sensing cells known as rods have been clearly and directly imaged in the living eye for the first time. Using adaptive optics (AO), the same technology astronomers use to study distant stars and galaxies, scientists can see through the murky distortion of the outer eye, revealing the eye's cellular structure with unprecedented detail. This innovation, described in two papers in the Optical Society's (OSA) open access journal Biomedical Optics Express, will help doctors diagnose degenerative eye disorders sooner, leading to quicker intervention and more effective treatments.

"While therapies are only emerging, the ability to see the cells you are trying to rescue represents a critical first step in the process of restoring sight," says researcher Alfredo Dubra of the University of Rochester in New York, who led the team of researchers from Rochester, Marquette University, and the Medical College of Wisconsin (MCW), Milwaukee. "It's impossible to overemphasize how important early detection is to eye disease."

"One of the major hurdles in detecting retinal disease is that by the time it can be perceived by the patient or detected with clinical tools, significant cellular damage has often already occurred," adds team member Joseph Carroll of MCW.

The breakthrough that is ushering in a new era of eye disease research, diagnosis, and treatment is an improved design of a non-invasive adaptive optics imaging system. Dubra and his colleagues were able to push the device's resolution to its optical limits of nearly 2 microns (a micron is 1/1,000 of a millimeter), or the approximate diameter of a single rod in the human eye.

Rods are much more numerous than cones and are vastly more sensitive to light. "This is a really exciting breakthrough," says Steve Burns, a professor in the School of Optometry at Indiana University, who is not involved in the Biomedical Optics Express research. "Imaging contiguous rod mosaics will allow us to study the impact of a whole new class of blinding disorders on the retina. Since many of the eye diseases most amenable to intervention affect the rods, this should become a major tool for determining what treatments work best for those disorders."

In astronomy, adaptive optics is able to correct for the blurring effect of Earth's atmosphere, effectively removing the "twinkling" from starlight and rendering cosmic objects as very sharp points of light. To achieve this correction, the AO system requires a reference point-either a bright, nearby star or an artificial "guide star" produced in the upper atmosphere by lasers mounted on a telescope. By monitoring that reference point, AO systems use a deformable mirror to create the exact but opposite distortion that is happening in the atmosphere. The result is a clearer image with much greater resolution.

Just as light passing through the atmosphere becomes bent and distorted, so too does light passing through the front part of the eye. This distortion is inconsequential on the scale of human vision, but poses a significant barrier in the microscopic realm of medical imaging.

In 1997, David Williams of the University of Rochester led the group that first demonstrated using AO technology to study the interior of the human eye. In this system, called an adaptive optics ophthalmoscope, a laser creates a reference point that is used to correct the blurring of the image obtained with a fundus camera. Today the fundus camera is commonly replaced by a second laser for imaging, which is known as an adaptive optics scanning laser ophthalmoscope. By moving the laser point across the retina and correcting the distortion along the way, line-by-line an accurate image emerges, in much the same way that a CRT monitor renders an image.

Though earlier AO systems could effectively image cones and have become a mainstay of high-resolution retinal imaging research, the smaller rods, which outnumber cones 20 to 1 in the retina, have eluded clear and contiguous observation in the living eye.

Breakthrough Design

The breakthrough in the design of the AO instrument that led to clearly visualizing rods was, according to Dubra, "embarrassingly simple, and relied on well-known equations and concepts." By simply folding the spherical mirrors that act as lenses in the instrument into a three-dimensional structure, the image quality of the retina was improved sufficiently to clearly resolve the contiguous rod mosaic, as well as the entire cone mosaic at the foveal center.

"By combining careful optical engineering, excellent adaptive optics control, and knowledge of the visual system the authors have made a major advancement in both biomedical imaging and vision science," says Burns.

Improving Patient Care

According to the researchers, their next step is to develop a clinical model that could be widely available. A related task is simplifying and teaching the art of interpreting AO images to guide clinical decisions about diagnosis and treatment.

When that occurs, hopefully in the next 5 to 10 years, doctors will likely be able to routinely peer into a living human eye with such precision and clarity that they will be able to see and evaluate individual rods-and do three things never before possible: accurately describe the physical presentation of specific rod disorders-the "phenotype" of a disease, intervene with early treatment at the first sign of disease, and even determine how individual cells are responding to a specific treatment.

"That's what's really exciting about this imaging device: it can really make a difference in a patient's life," says Carroll. "The ability to now resolve these cells opens up new possibilities for improving care that researchers have been anticipating for a long time-such as using the information in these retinal images to aid in targeting, delivering, and evaluating therapies."

The rod photoreceptors are implicated in a number of devastating retinal diseases. However, routine imaging of these cells has remained elusive, even with the advent of adaptive optics imaging. Here, we present the first in vivo images of the contiguous rod photoreceptor mosaic in nine healthy human subjects. The images were collected with three different confocal adaptive optics scanning ophthalmoscopes at two different institutions, using 680 and 775 nm superluminescent diodes for illumination. Estimates of photoreceptor density and rod:cone ratios in the 5°-15° retinal eccentricity range are consistent with histological findings, confirming our ability to resolve the rod mosaic by averaging multiple registered images, without the need for additional image processing. In one subject, we were able to identify the emergence of the first rods at approximately 190 μm from the foveal center, in agreement with previous histological studies. The rod and cone photoreceptor mosaics appear in focus at different retinal depths, with the rod mosaic best focus (i.e., brightest and sharpest) being at least 10 μm shallower than the cones at retinal eccentricities larger than 8°. This study represents an important step in bringing high-resolution imaging to bear on the study of rod disorders.

Interview with Dr Loren Fishman on Yoga for Osteoporosis and Other Topics

For those of you not familiar with the work of Dr. Fishman, he is a long-time yoga practitioner, a physician specializing in rehabilitative medicine, and someone who is endlessly curious about the power of yoga to address health issues, such as osteoporosis, osteopenia, arthritis, and scoliosis. We are fortunate he agreed to this interview today so we can learn more about his recent research on bone health.  

Baxter: Can we begin with a little background on what got you interested in osteoporosis and yoga in the first place?  

Loren: When I was in India (1972-75) I got the first part of the idea, seeing these people from a truly ancient culture who still walked hundreds, even thousands of miles. These people seemed to me to have such strong, almost metallic bones. Mr. Iyengar gave me the same impression, only more so. 

Afterwards, in medical school, when I learned about Wolff’s law, that the strength of a bone depended on the forces to which it was subjected, I put the puzzle together: Yoga, generating forces many times greater than gravity, built bones up and made them stronger. But it has to be yoga graced by great attention to alignment. Otherwise, those great forces would cause injury.  

Baxter: We have written about your early work on osteoporosis in a pilot study done in 2009, but I know you have done a more recent study with exciting results. Can you tell us about that research and how our audience of yoga practitioners and yoga teachers might use what you have discovered?

Loren: After the pilot study was successful, I made a DVD and gave it away to 1000 people from every part of the world to use for our next study. It took eight years to get all the data we needed to complete this second study. We published this 1000 patient study (241 actually compliant) as Twelve-Minute Daily Yoga Regimen Reverses Osteoporotic Bone Loss, Lu, Yi-Hsueh; Rosner, Bernard; Chang, Gregory; Fishman, Loren M. Topics in Geriatric Rehabilitation April/June 2016 - Volume 32 - Issue 2 - p 81–87.  

Baxter: Can you tell us about the safety and accessibility of your bone strength protocols?

Loren: Safety? We asked everybody in the 1000-patient study to get X-rays before and after the two years they practiced using the DVD. About one quarter of them actually did. Results: No fractures, no significant advance of arthritis. No yoga-related fractures reported by anyone in the study. In fact no serious injuries of any kind were seen or reported in over 100,000 hours of people doing this yoga daily. Over 80% of them had osteoporosis or osteopenia when we started. In the final analysis, the spine and hip improved significantly, (p =.02 and.01), and the femur improved more than it does in people using the most popular medicines (p = .0002).

Baxter: Are there any unanswered questions you still have regarding the use of yoga for osteoporosis? 

Loren: Yes. How do you convince the doctors? The study got written up in the New York Times, and the one on scoliosis in the Wall Street Journal, but the medical community remained largely unpersuaded. We are now working on another study that we call a dose-response study, in which we have participants see a yoga teacher who is trained in our yoga every two weeks to see what “dose” of yoga the participants are giving themselves. Then we’ll see what happens to their DEXA scans in two years. I suspect not only does it work, but is synergic with the various medicines, yielding an effect that is greater than just adding the two effects together. But that’s just a suspicion. In a few years or so we’ll see if there’s a relation between how much you put into your yoga and how much stronger your bones become. 

Baxter: What is next for you? Any upcoming or ongoing studies? And can our readers participate in some way?

Loren: Yes, we’re now doing another certification training in osteoporosis, and certified teachers can give classes, report data, and participate in our newest dose-response study. We also have one coming up on lower back pain. After all, back pain is the biggest medical reason people seek out yoga. Here yoga appears synergic with medicine. These courses are analogous to “post graduate work” after becoming a yoga therapist. They have online and workshop components.

We’ve just gotten another study accepted by “Muscle and Nerve” on botulinum toxin for piriformis syndrome. And we just submitted a study in which yoga is more effective for scoliosis than any conservative method ever published, and unlike surgery, innocuous and just about free. We have patients from age 8 to the late 80’s. Then we’re working with Carnegie Mellon’s computer people on a new, objective way to analyze the facial expression of pain, to give a more objective way to understand pain. Do people realize that probably one third of medicine is based on pain, and yet all we have to rate it is the puerile one-dimension 1-to-10 scale?  

Baxter: I understand you have started offering trainings in your way of addressing osteoporosis. What are these trainings like and where can our readers learn more about them?

Loren: The up-and-coming trainings are in North Carolina, at an Appalachian mountain retreat called Art of Living near Boone, starting May 31, and another in San Francisco in mid-July. That San Francisco training is almost full, but the one in Boone is pretty open. They are 40-hour teachings, 20 hours online at artoflivingretreatcenter.org.


We have also developed alternative sets of poses for osteoporosis to ward off the inevitable ennui of doing the same poses day in day out, and also to strengthen the bones of the wrist, the fourth most probably fracture site (as I said earlier, we have put the first group—the group we’ve already proven to work—online for free at YouTube and offer DVDs on the website sciatica.org). And we give workshops, such as the one at Art of Living in North Carolina and online training to teach yoga teachers to train people to do these poses at their proper level. We feel this is an effective, innocuous, and quite valuable skill for yoga teachers to acquire.

Baxter: Is there anything else you’d like to say to our readers?  


Loren: Yes, there’s a recent controversy about how long should a yoga class be, based on an article in the Atlantic Monthly. In this era of Dog Yoga and Pizza Yoga I think people can get on the wrong track. Like anything else, the length of a class depends on whom you’re teaching and what you’re teaching them. The length isn’t important in itself. How long does a word have to be to keep its meaning? One should measure progress not by distance traveled, but by proximity to goal.  

Baxter: Sage advice, and thank you for all of this important and relevant information. We look forward to hearing more of the results of your studies in the future!
